Chronic
Care
Management

Any long-lasting condition that can be managed, but not completely cured, is a chronic disease. Patients with chronic conditions typically have a higher rate of hospitalization and more visits to the emergency room. Fortunately, people with two or more chronic diseases qualify for Chronic Care Management, a program approved by the Centers for Medicare and Medicaid Services (CMS). Examples of chronic diseases include arthritis, asthma, heart disease, and depression.

CHRONIC CARE MANAGEMENT (CCM) PROGRAM:

When patients with chronic conditions actively participate in their healthcare, their overall care coordination and outcomes improve, meaning fewer hospitalizations and trips to the emergency room. Patients of the CCM program receive:

Medication Management
We offer assistance with medication reconciliation and self-management of medications. Patients suffering from chronic diseases take multiple types of medication each day, leading to confusion and the possibility of mistakes during the medication process. Those in the CCM program can rest easy knowing that St. Thomas helps with medication management.

Clinical Services Coordination
Many patients in the CCM program work with different doctors to help manage their chronic diseases. St. Thomas coordinates with home- and community-based clinical services that are needed, so patients know who they are being treated by and how often they need to make an appointment.

Treatment Goals
When you suffer from chronic diseases, you don’t want your symptoms to pile up. Under the CCM program, patients and healthcare providers create and maintain a problem list, which includes expected outcomes for each ailment and measurable and attainable treatment goals.

Ongoing Education
Diseases and treatments are constantly changing. At St. Thomas, we stay on the cutting edge of treatment options while educating our patients on new ways to stay healthy and active. Patients in the CCM program consult with their doctors and healthcare providers on a regular basis to understand how best to manage their illnesses.
