Apply for a Scholarship

A limited number of scholarships will be available for individuals who plan to attend both days of the symposium. These scholarships are for two nights lodging at the conference hotel.

Information provided on your submission is NOT confidential and will be shared with a committee that reviews all requests.

The planning committee will make a hotel reservation for each scholarship recipient. To receive two complimentary nights of lodging at the conference hotel, recipients must confirm their arrival and departure dates as well as provide a credit card guarantee upon notification of scholarship acceptance.

Though all staff members and administrators who meet the above criteria are invited to apply, only a maximum of three scholarships per agency will be awarded. Scholarships will be awarded based on financial need, geographic distribution, and population(s) served. Also considered are the skills the participant will be able to use in his/her agency and community. Applications will be accepted from primary care center staff working in Louisiana only.
